Divide the nursing care plan into five parts: Assessment, Diagnosis, Planning, Implementation, and Evaluation. Take 2-3 hours to write all the subjective and objective information about your patient. This includes the patient’s response to certain medications, lab reports, and vital signs. Invest 3-4 hours in examining the diagnosis based on the standard Nursing and Midwifery Council guidelines. Allot 3 hours to planning interventions using the SMART rule: Specific, Measurable, Achievable, Realistic, and Time-bound.
